A Clinical Study of Total Gastrectomy for Stomach Cancer

Abstract
Over a 5-year period from Jan. 1977 to Dec. 1981, 18 patients with gastric cancer underwent total gastrectomy at KCH and analysed them. Following results were obtained.
1) Gastrectomy was performed in 260 patients and 71 patient(44.5%) and 18 patients (11.2%) of them were performed radical subtotal gastrectomy and total gastrectomy respectively.
2) The highest age group was the 6th decade(44.4%).
3) The symptoms were epigastric discomfort, indigestion, neusea, vomiting and weight loss.
4) The location of tumor was body of the stomach 6 cases (33.3%), cardia of the stomacti 6 cases(33.3%), diffuse infiltration of gastric wall 5 cases(27.7%), and multiple location of gastric cancer 1 ca.se(5.5%).
5) The esophagointestinal continuity was end to side esophagojejunostomy with Braun anastomosis in 5 cases(27.70/,) and Hunt-Lawrence jujunal pouch with Roux-en-Y jejunojejunostomy in 13 cases(72.3%).
6) 6 cases of postoperation complications occured in 4 patients. The complications were followed; wound infection 2 cases(11.1%), ileus 1 case.(5.5%), pleural effusion lcase(5.5%), esophagojejunal leakage 1 case(5.5%) and hepatorenal syndrome 1 case(5.5%)
7) One year and two year survival rate were 55.5% and 18.1% respectively and postope-rative motality was 5.5%(1 case).
